例例声辉｜不可切除局晚期NSCLC在放化疗的基石上加一剂猛药( 二 )

目前抗血管生成治疗联合放化疗是治疗局部晚期NSCLC的一种新的探索模式。 HELPER研究显示[6] ，相较于既往依托泊苷/顺铂（EP）同步放疗，持续静脉泵注重组人血管内皮抑制素联合EP方案+同步放疗大大提高Ⅲ期不可切除NSCLC患者生存，中位总生存期（OS）和客观缓解率（ORR）分别为34.7月和76.1% 。
2020年Radiation Oncology发表的一项汇总分析[7] ，再次证实重组人血管内皮抑制素联合放疗（ERT）或同步放化疗（ECRT）对于局部晚期NSCLC患者的良好疗效和安全性。该汇总分析结果显示， ECRT、ERT组患者ORR分别为77.5%和76.1% ，中位无进展生存期（PFS）分别为11.2个月和11.8个月，中位OS分别为18.4个月和19.6个月，且不良反应在可控范围之内。
本例ⅢC期不可切除NSCLC患者起始治疗应用重组人血管内皮抑制素联合化疗，实现肿瘤部分缓解；随后患者接受重组人血管内皮抑制素联合放化疗，肿瘤维持部分缓解状态，证实了同步放化疗联合重组人血管内皮抑制素是不可切除局晚期NSCLC患者的一种有效且安全性良好的治疗方案。
